What is the BCG vaccine?

BCG (Bacillus Calmette-Guérin) is a live attenuated vaccine against tuberculosis (TB). It is given as a single intradermal injection, usually into the upper left arm. In the UK it is recommended for newborns at higher risk of TB - primarily those in families from countries with high TB prevalence - and for healthcare workers without documented prior immunity.

Recommended for…

Newborns in families from high-TB countries, children moving to high-TB areas, healthcare workers, anyone at occupational risk.

✓

Newborns at higher TB risk

Families from countries with TB rates above 40/100,000 - recommended in the first few weeks of life.

✓

Moving to a high-TB country

Children planning to live or travel for extended periods in high-prevalence countries should be vaccinated first.

✓

Healthcare workers

All UK healthcare workers without documented BCG immunity should be vaccinated before patient contact.

Frequently asked questions

How is BCG given?
As a single intradermal injection into the upper left arm - not a subcutaneous or intramuscular injection. A small bleb forms at the injection site.
Does my baby need a skin test first?
Children under 6 weeks do not need a Mantoux test first. Older children and adults should have a Mantoux (tuberculin skin test) before BCG to ensure they have no prior TB immunity.
What happens at the injection site?
A small papule develops over 2–6 weeks, may ulcerate, and usually heals leaving a small scar. This is normal and expected.
How long does BCG protection last?
BCG provides approximately 15–20 years of protection against severe TB, including TB meningitis in children. It is generally given once; repeat vaccination is not recommended.
