What is ambulatory blood pressure monitoring?
Measuring your blood pressure is a simple way of checking whether it is too high or too low. High blood pressure can lead to heart failure and strokes - making this test potentially life-saving.
Ambulatory blood pressure monitoring (ABPM) is far more accurate than isolated single readings taken in a clinic. The A&D TM-2441 monitor records your blood pressure automatically over a full 24-hour period, enabling you to move around and continue with your normal daily life.
NICE-Recommended Gold Standard
NICE guidelines (NG136, updated 2025) recommend ABPM as the first-line investigation to confirm hypertension - more reliable than any single clinic reading.
Automatic Every 30 / 60 Minutes
The cuff inflates automatically every 30 minutes during the day (07:00–22:00) and every 60 minutes overnight - giving approximately 48 valid readings across the full 24 hours.
Why might I need blood pressure monitoring?
Your doctor may recommend ABPM for several important reasons. Measuring your blood pressure at regular intervals over 24 hours provides a far fuller picture than any single clinic reading.
White Coat Syndrome
Some patients experience anxiety in clinical settings, causing a temporary spike that doesn't reflect their true baseline. ABPM captures readings in real life, removing this bias entirely.
Confirming Hypertension
NICE recommends ABPM to confirm a clinic reading of 140/90 mmHg or higher before starting treatment - preventing unnecessary medication based on a single elevated reading.
Assessing Medication
If you're already on blood pressure medication, ABPM objectively shows how well your treatment is working across the full 24 hours, including whether control is maintained overnight.
Nocturnal Hypertension
High blood pressure during sleep is a serious risk factor often completely missed by daytime clinic readings. ABPM captures your overnight pattern to detect this hidden danger.
Masked Hypertension
Normal readings at the clinic but elevated blood pressure during daily life - masked hypertension significantly increases cardiovascular risk and can only be identified with ABPM.
Variable Clinic Readings
If your blood pressure readings are inconsistent during clinic visits, ABPM gives a reliable, averaged picture across the full day, helping direct accurate treatment decisions.
What happens during ABPM?
ABPM requires two clinic visits - one to fit the A&D TM-2441 device, and one to return it the following day for data download and analysis.
Visit 1 - Device Fitting
Attend our clinic to have the cuff fitted to your upper arm and the monitor attached to your waist belt or shoulder strap. A test reading is taken to confirm correct placement. Takes 15–20 minutes.
Wear for 24 Hours
The device inflates automatically every 30 minutes during the day and every 60 minutes overnight. Carry on normally, keep your arm still during readings, and note events in your diary.
Sleep with It On
Keep the monitor on overnight - nocturnal readings are a critical part of the assessment. Rest the cuffed arm by your side or on a pillow. Avoid lying directly on the cuff.
Visit 2 - Return & Results
Return to our clinic the next day to have the device removed. Data is downloaded via our ABPM Data Manager software and a full clinical report is prepared and sent to your GP.

Paying for your test
Our ABPM service is private and self-pay. We do not bill insurers directly, but we provide a fully itemised invoice you can submit to your insurer to claim the cost back yourself.

Our clinical team will explain everything at your fitting appointment. Here is what to know before you come in, and how to get the best results during your 24-hour recording.
A Before you come to have the monitor fitted
Wear a loose-sleeved top
Wear a loose short-sleeved top, or a thin long-sleeved top, so we can fit the cuff on your upper arm easily.
Have a normal day
You do not need to fast or change your plans unless your clinician has specifically advised you to do so.
Continue your usual medication
Take all medications as normal unless your clinician has specifically told you to do something different.
B When the cuff starts to inflate
Stop and keep still
Stop what you are doing if you can. Keep your arm straight and relaxed by your side, with the cuff at about heart level. Do not talk while the cuff is inflating and deflating.
Sleeping with the monitor
Try to sleep in your usual position. Keep the cuffed arm relaxed by your side or on a pillow. Avoid lying directly on the cuff. Keep the monitor pouch safely beside you so the tube is not pulled tight.
C Dos and don'ts
- ✓ Continue normal daily activities as much as possible
- ✓ Keep the monitor on for the full 24 hours including overnight
- ✓ Keep the cuff and monitor completely dry
- ✓ Carry the monitor in its pouch and keep the tube straight
- ✓ Write in the diary - bedtime, wake time, medicines, symptoms, unusual stress
- ✗ Get the monitor or cuff wet - avoid showers and baths while it is on
- ✗ Remove the cuff or monitor unless told to by the clinic
- ✗ Press buttons on the monitor unless shown how to by our nurse
- ✗ Do heavy arm exercise (e.g. weight training) on the cuffed side
- ✗ Drive with the cuffed arm on the steering wheel if inflations distract you

ABPM requires two clinic visits. The first visit is to fit the A&D TM-2441 device (15–20 minutes), and the second visit the following day is to return and remove the device, after which the data is downloaded and your report prepared.
The device inflates approximately every 60 minutes overnight, which some patients find mildly noticeable. Most people adapt quickly. Try to keep the cuffed arm relaxed by your side or on a pillow and avoid lying directly on the cuff.
After returning the device at your second visit, data is downloaded and analysed using our ABPM Data Manager software. A comprehensive clinical report is prepared and sent to your nominated GP, typically within 3 working days.
Contact our clinic during working hours on 020 8861 1221 if the cuff keeps slipping, becomes very painful, or the monitor shows an error. For emergency symptoms (severe chest pain, breathlessness, sudden weakness) - seek urgent medical help immediately, do not wait because you are wearing the monitor.
Yes - if your symptoms suggest both a blood pressure concern and a heart rhythm issue, both monitors can be worn simultaneously. Please mention this when booking and our clinical team will advise on the combined approach.
